General Problem

Despite extensive research over the recent years in the healthcare industry, a knowledge gap still exists when widespread changes in hospital levels over time is considered. Clinicians in their endeavor of trying to identify why some institutions perform better than other healthcare facilities have provided insight on roles played by different factors.

Studies conducted for the commonwealth fund by shows that hospitals which are regarded as top notch can be distinguished from the others through culture, designs applied to attract and retain loyal customers, methods which healthcare practitioners update and devise in-house processes for improvement in quality and the tools provided to the staff for performing their duties.(Hazinki, 2012). From a survey performed on chief executive officers for the top 100 national hospitals, characteristics for high performance personnel in recruitment, retention and culture development was identified. It was evident that lack of adoption of adequate strategies in nursing such as pay for performance, shared governance and the chase for magnet status resulted to lower patient output.

Aim Statement

By the development of the essay-based practice, my main aim will involve providing methods which will help in improving process operations. This essay will aim to reduce delays in test results within the first three months of the implementation of the essay recommendations. It will also aim to increase patient flow rate by 40% within the first four months of its induction. Efficient data sharing and recording schemes will be implemented in the essay based practice which will reduce in medication errors by 50%. In health related improvements, improvements in healthcare practices will aid in mortality rate reductions, reduction in blood infection cases by 30% of the total surrounding population, reduced complications after treatments by 30% in one month, reduction in patient re-admissions and patient falls by 60% within 12 months. The work environment improvements that would be inferred from the essay will include patient satisfaction increase through provision of favorable reviews, staff morale boost by 50% in two months and improved ability in attracting quality staff and physicians.

Preliminary Literature Review

In the recent years, sub-par qualities in healthcare professionals has been extensively documented in different forms of literature. This has resulted in foundations, federal governments and private sectors funding researches which identify appropriate clinical practices through developing strategies which reduce medical errors therefore resulting to improvement of health outcomes.

From Hazinki (2012), in the institute of medicine (IOM) reports, “To err human and crossing the quality chasm” studies were performed to determine how quality within healthcare facilities could be effectively measured. The most ambitious study within the reports was the “High performance special study” (HPSS) which was supported by the CMS (centers for Medicare and Medicaid services). The HPSS study aimed to develop and implement a methodology which could be used in defining quality performance and the identification of hospitals with stellar performances. Characteristics and practices which would distinguish them from other hospitals was also identified. Quantitative performance data based on AMI (acute myocardial infractions), CHF (congestive heart failure) and pneumonia, were used to identify a countrywide list of high performing and non- performing hospitals. Interviews were conducted by the researchers on 110 key informants through six pairs of non-high and high performing hospitals. Differentiation of common models for quality improvements was performed through various criteria which included technology, culture, responsibilities, targets and priorities. Within these models, identification of specific high-leverage and basic change ideas was performed as a mode of completion. From the data collected, the authors concluded that the achievement of high quality levels in hospitals usually requires an effective approach which links dimensions of improvement, communication, clinical management strategies and monitoring.

According to Fouton and Gouldreu (2014), “Foundations of clinical nurse specialist practice” major disparities from healthcare practitioners were identified. Data collection for the research was not only performed across different geological regions and different hospitals but was also performed in hospitals with various disease states with an existence of different conditions. The research team identified 10 measures which reflected quality care for pneumonia, CHF and AMI. Inconsistency in quality within the institution would raise questions on how to assess the quality of a hospital and identification of how best practices could be transferred from a well performing department to a department with medial patient output.

Implementation of quality improvement initiatives when improving patient outcomes is through the use of evidence-based practices. This works of literature provides information on the regions which improvements are required through provision of a quantitative analysis of hospitals in relation to the identified factors. It was evident that in successful implementation of protocols, organizational support for change constitutes the most significant factor.
